Welcome to the 2014/15 Wiltshire Care Services Directory which provides information about the range of social care services available in Wiltshire.

At Wiltshire Council we’re committed to supporting vulnerable adults and older people. This is why  we’re investing in care services which help you live independently in your own homes and communities for as long as possible.

Supporting you to live independently in your own homes underpins our Help to Live at Home (HTLAH) programme. Since it was introduced in 2011, it has resulted in a number of new services which have been developed by local people, care providers and front-line staff. This has ensured the services we provide are focused on what you need and want. We understand that for some of you living in your own homes is simply not an option and for many of you, living in supported accommodation or a care home is your preferred choice. Therefore, this Directory contains  useful information about care homes which will help you choose the right home for your needs.  It also provides details of services that support those of you who care for others.

The Council has appointed/identified a number of partners to deliver a range of services to you in your home. These companies are: Medvivo who provide a response service linked to telecare (assistive technology); Telecare; community equipment and practical help in the home provided by Medequip, independent support planning and brokerage services provided by the Wiltshire  Centre for Independent Living and specialist financial advice provided by Eldercare Group and Ashcourt Rowan. Details of these organisations and the services they provide can be found in this Directory.

In addition four companies have been chosen by the Council and NHS to provide care and support services to older people and others who need some extra assistance to ensure they can continue to live the lives they want as independently as possible.
